1. Technical Competence: You should talk about any technical skill/profession you might have.

2. Quality and Accuracy: You should talk about work quality your perform and how accurate you are to finish the job

3. Dependability: You should talk about yourself’s dependence or independence; Do you rely on other to finish your job or not?

4. Communication: You should talk about methods of communication with your colleagues and inside the company. How do you communicate?

5. Office discipline: You should talk about how much you follow office discipline and rules.

6. Drive and initiative: You should talk about your participation in doing things and your capability of performing. About what drives you (or motivate you).

7. Improvement: You should talk about how much you got improved and in what you got improved. Mention your improvement’s points.

8. Overall effectiveness: You should talk about you overall effort and performance in the company/organization


- Section B: Developmental needs

Here, you should mention if you need or you require any developmental needs like training, orientation, support...Etc

- Section C: Appraisee’s comment

You should write down any comment, remark or suggestion you like to share with the appraiser.

- Section D: Remarks by appraisee on his/her colleagues

Write down any remark about your co-worker or colleague in the company/organization, whether good or bad remarks.
