After the first several meetings, team members should anonymously complete this evaluation of their team, and afterwards discuss methods for improvement.

Section 1: Use the following 5 alternatives in answering the questions on communication and problem-solving:
1-Strongly disagree 2-Disagree 3-Neutral 4-Agree 5-Strongly agree

Communication:
___ All team members listen to each other.
___ All team members are open and candid.
___ All team members check things out before taking action.
___ Constructive feedback is given freely to improve team functioning.

Section 2: Rate your team on a 1-5 scale, with 5 meaning “perfect, no room for improvement”, and 1 meaning “poor, needs a great deal of improvement.”
